We need a place to be able to add notes regarding open tasks that can be displayed when we view all open tasks.

This is important in managing all open items for a given user. As a manager I can review notes to understand why a task may still be open. As a staff member managing my open tasks I can leave notes for why something may not be completed or for things I need to get from a client or do before I complete the task.
